The following is a community resource document created by Comunidades Unidas on information about clinics, medical centers, emergency rooms, and blood donation.

It is recommended to call before going in as the COVID-19 pandemic could have changed some policies at these centers.


Utah Partners for Health / Mid-Valley Health Clinic 

  • 7651 S Main St, Midvale, UT 84047, USA (801) 417-0131.

  • Open: Mon-Wed: 8 AM - 5 PM. Thurs: 12 PM - 8 PM. Fri 8 AM - 5 PM.

  • Utah Partners for Health Sites: Medical and Behavioral Health 8446 S Harrison Street, Midvale, UT 84047. Medical 9103 S 1300 W Suite 102, West Jordan, UT 84088. Dental 7651 S Main Street, Midvale, UT 84047 


Mobile Clinic (Utah Partners for Health)

  • 8446 S. Harrison St. Midvale, UT. 84047 TW. 8 AM - 2 PM (Medicine and Vision) Thursday. 1 PM- 6:30 PM (Just Medical) (385) 204-6257 (385) 212-4802.

  • Requirements: Photo ID. Just Medicaid or medicare. Have an annual income of less than 200% of the Federal Poverty Level.


Intermountain North Temple Clinic

  • 54 N. 800 W. SLC.UT.84116 Mon-Sat 10 AM-7 PM (801) 408-8654.

  • Requirement: low income, uninsured, or insured. Spanish speakers/translators are available. Sliding scale cost. 

  • Only programs offered are for urgent care.


Maliheh Free Clinic

  • 415E. 3900S. SLC, UT 84101 Monday through Thursday from 9 a.m. at 12 p.m., from 1 to 4 p.m. Tuesday and Wednesday nights 4 PM- 8 PM. Fri 9 am-12pm (801) 266-3700.

  • Requirement: No insurance, proof of income and household income below 150% of the Federal Poverty Level.

  • Spanish speakers/translators are available.


Hope Free Clinic

  • 65 E. 6850 S. Midvale, UT. 84047

  • Tuesdays and Wednesdays from 8 am to 4 pm (801) 568-6700.

  • Requirements: uninsured and 150% below poverty level.

  • Spanish speakers/translators are available. General cost: free clinic. 


Ellis R. Shipp Community Health Center

  • 4535 S. 5600 W. WVC, UT. 84120

  • Monday through Friday 7:00 AM- 6: 00 PM. Wednesday 8 AM- 6 PM. (801) 679-4405

  • Requirement: Insured or uninsured people need one month's income verification.

  • Spanish speakers/translators are available.

Midvale (CBC) Community Clinic

  • 49 W Center St, Midvale Ut 84047 Mon-Fri. 8 am-5pm (385) 887-9002.

  • Requirements: Uninsured and formal identification (license plates accepted), proof of income.

  • General Cost: An initial $15 copayment is required for all services.


Southeast Public Health Center

  • 9340 S. 700 E. Sandy, UT. 84070 Mon-Fri 8:00 AM - 5:00 PM. (385) 468-4330.

  • Requirements: Can be insured or uninsured. New patients are accepted and it is an immunization clinic.


72nd Street Community Health Center (CHC)

  • 220 W 7200 S. Midvale UT, 84047 Monday through Friday. 8:00 AM- 5:00 PM. (801) 566-5494.

  • Requirement: Insured and uninsured are welcome and one month's income verification is required


Oquirrh View Community Health Center (CHC)

  • 4745 S. 3200 W. Taylorsville, UT. 84118 Monday, Tuesday, Thursday, Friday 7 am - 6 pm; Wed 7 am - 7:30 pm (801) 964-6214.

  • Requirement: Insured and uninsured are welcome and one month's income verification is required. Spanish speakers/translators are available.

Central City Community Health Center (CHC)

  • 610 S. 200 W. Suite B, SLC, UT Mon, Tue, Thu, Fri 7 am - 6 pm; Wed 7 am - 7:30 pm (801) 964-6214.

  • Requirement: Insured and uninsured are welcome and one month's income verification is required.

  • Overall Cost: Based on income and using a sliding scale.


Sacred Circle Healthcare

  • 660 S. 200 E. Ste. 250, SLC, UT. 84111 Mon - Fri: 8:00 AM - 6:00PM. (801) 359-2256.

  • Requirements: preferred patients with Medicaid, medicare. 


Intermountain Clinic Lincoln Elementary

  • 660 S. 200 E. Ste. 250, SLC, UT. 84111 Monday through Friday from 8:30 a.m. at 4:30 p.m. (801) 578-8376.

  • New patients are accepted through appointments.


Midtown South Salt Lake CHC Clinic

  • 2253 S. State St. SLC. UT 84115 Mon-Fri. 7:30 AM – 6:00 PM. Closed for lunch 12:45 P.M- 1:45 P.M (801) 486-0911.

  • Requirements: insured and uninsured are welcome.

  • One month income verification and photo ID required. Spanish speakers/translators are available.

  • Overall Cost - Based on income and a sliding scale.


U of U Westridge Health Center

  • 3730W. 4700 S. WVC, UT. 84129 Monday 7 a.m. to 7 p.m., Tuesday to Friday from 7 a.m. to 5 p.m., Saturdays and Sundays from 9 a.m. to 9 p.m. (801) 213-9200.

  • Requirement: Most insurances are accepted and the uninsured are welcome. Spanish speakers/translators are available.
